The best time to buy a Medigap (Medicare Supplement) policy is during your Medigap Open Enrollment Period. This 6-month period begins on the first day of the month in which you’re 65 or older and enrolled in Part B. (Some states have additional Open Enrollment Periods.) After this enrollment period, you may not be able to buy a Medigap policy. If you’re able to buy one, it may cost more.

** If you enroll in a Senior Security or Senior Preferred plan at age 65 through 72, you receive an early enrollment discount on your rate. The early enrollment discount is reduced annually through the age of 76. The change in discount occurs in the next month's bill after your birthday. When your discount no longer applies, you will be charged the BlueValueSM or Standard rate assigned to your Senior Security or Senior Preferred plan. Medicare Supplement 5% discount available to households with two or more Medicare Supplement members. Certain terms and conditions apply.

The TruHearing program is not an insurance policy and does not provide insurance coverage. One year of follow up visits with a TruHearing provider must be used within 12 months after the date of initial purchase. Free battery offer is not applicable to the purchase of rechargeable hearing aid models. Three-year warranty includes repairs and one-time loss and damage replacement. ScriptSave® WellRx Premier is a drug discount card program operated by Medical Security Card Company (MSC), LLC (dba ScriptSave) of Tucson, AZ, an independent and separate company contracted with BCBSAZ to make the ScriptSave® WellRx Premier Prescription Savings program available to BCBSAZ members. The ScriptSave® WellRx Premier program is not insurance coverage. MSC does not provide BCBSAZ products or services and is solely responsible for ScriptSave services. DISCOUNT ONLY — NOT INSURANCE. Discounts are available exclusively though participating pharmacies. The range of the discount will vary depending on the pharmacy or provider chosen and services rendered. The program does not make payments directly to the pharmacies or providers. Members are required to pay for prescription purchases and any other discounted services used. You may cancel your registration at any time or file a complaint by contacting Customer Care at 1-800-700-3957. Average savings of 54%, with potential savings of up to 80%. All prescription medications are eligible for savings.

Services and treatment options presented may not be covered under your BCBSAZ benefit plan. BCBSAZ contracts with a third party to administer Nurse On Call services. The Nurse On Call service should not be used in health emergencies. If you have a health emergency or need immediate help for an accident or injury, seek emergency care or call 911. Blue Cross® Blue Shield® of Arizona (BCBSAZ) is contracted with Medicare to offer HMO and PPO Medicare Advantage plans and PDP plans. Enrollment in BCBSAZ plans depends on contract renewal.

BCBSAZ offers BluePathway HMO and BlueJourney PPO Medicare Advantage plans. BCBSAZ Advantage, a separate but wholly owned subsidiary of BCBSAZ, offers Blue Medicare Advantage Standard, Classic, and Plus HMO plans.

You are eligible to enroll in a BCBSAZ Medicare Supplement plan if you are age 65 or older, entitled to Medicare Part A, and enrolled in Medicare Part B, and you live in the plan service area. You must continue to pay your Medicare Part B premiums (and Part A, if applicable), if not otherwise paid for by Medicaid or another third party. During the first six months when you are age 65 and also enrolled in Medicare Parts A & B you cannot be denied a Medicare Supplement plan when you apply for one, regardless of health status.

Health Choice Pathway HMO D-SNP is a Health Plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in Health Choice Pathway HMO D-SNP depends on contract renewal. Health Choice Pathway Member Services can be reached at 1-800-656-8991, TTY: 711, 8 a.m. to 8 p.m., 7 days a week.
