Medication Therapy Management
The Medication Therapy Management Program is a no-cost service that helps members use their medications safely and effectively, and minimize the amount they spend on prescriptions.
Members who are eligible for this voluntary program will be contacted by us directly. You may be eligible for this program if you meet one of the following:
• Take eight or more Part D-covered maintenance drugs
• Have three or more long-term health conditions, such as high blood pressure, chronic obstructive pulmonary disease (COPD), or diabetes
• Have
$4,935
in annual drug costs paid by you and the plan in
2023
